About Us

At Trinity we are dedicated to providing progressive outpatient mental health rehabilitative services to youth ages five to twenty with behavioral, emotional, and social problems as well as adults twenty-one and older with serious mental illness.


Our Staff consists of highly trained professionals with years of experience in the mental health and social service fields, who are dedicated in assisting the population to achieve their maximum level of independent functioning. It is the policy of this agency to accept recipients without regard to race, sexual orientation, disability, religion, and or origin.

24 Hour Crisis Services

In Home, Office, and School therapy sessions are available.


  • Assessment, Reassessment & Psychiatric Evaluation
    • Obtain further information about mental health problems, such as anxiety, depression, schizophrenia, etc.
    • Help tell the difference between mental and physical health problems.
    • Evaluate a person who has been referred for mental health treatment because of problems at school, work, or home.
    • Evaluate the mental health of a person who has been hospitalized or arrested for a crime, such as drunk driving or physical abuse.

  • Development of Individual Treatment Plan
    • Defines in a person's own terms what they want to work on by breaking down measurable goals into objectives and interventions that will be used to acheive what they desire behaviorally to return to normal functioning.

  • Psychosocial Skills Rehabilitation (PSR)
    • A service delivery model utilized by licensed community based organizations that is designed to help individuals with persistent and serious mental illness develop the emotional, social, and intellectual skills needed to live, learn and work in the community in the least restrictive setting.

  • Community Psychiatric Supportive Treatment (CPST)
    • Provides an array of services delivered by community based and/or multidisciplinary teams of professionals. Services address the individual mental health needs of the consumer. CPST services are provided to individuals of all ages as well as families, and are provided in a client centered manner.

  • Group Counseling/Pass-Thru Services
    • Allows for the individuals to meet independently or in a group setting to discuss problems and concerns on a short term basis. Usually meets for one hour weekly for an interval period of time.

  • Medication Management
    • Is the monitoring of medications that a patient takes to confirm that he or she is complying with a medication regimen, while also ensuring the patient is avoiding potentially dangerous drug interactions and other complications.

  • Parent/Family Intervention
    • Provides parents, family members, caretakers open communication and coordination with individuals served to help reach treatment goals.

  • Referral & Linkage Services
    • Assisting individuals to locate other agencies or organizations that are more specialized and better equipped to meet their mental health needs.

  • Crisis Intervention
    • Is an emergency psychological care aimed at assisting individuals in a crisis situation to restore equilibrium to their biopsychosocial functioning and to minimize the potential for psychological trauma. The priority of crisis intervention and counseling is to increase stabilization.
